Healthy Start Care Coordinator
(Contracted 1099)

Job description

  • The Coordinator shall perform Healthy Start care coordination and wraparound services as set forth in Section 64F-3, Florida Administrative Code, Healthy Start Standards/ Guidelines and Pathways system of care.

  • The Coordinator will address the health and wraparound service needs of pregnant women, interconceptional women as identified in the HSSG, and children from birth up to age three who reside in Seminole County.

  • The Coordinator shall provide care coordination and wraparound services that promote healthy pregnancies, improved birth outcomes, and ensure that children from birth up to age three have access to services that promote normal growth and development. The coordinated care will maximize public and private cooperation, be cost-effective, minimize barriers to care, and promote improved health and consumer satisfaction to the extent of available resources. The care coordination services shall be developed in accordance with the procedures and requirements referenced in Chapter 64F-3, Florida Administrative Code and the Healthy Start Standards and Guidelines. These systems shall be implemented and maintained in the Coordinator's approved service area that is comprised of the following county: Seminole

  • Manage a caseload; organize and prioritize work assignments; make decisions and independent judgments; determine the appropriate course of action in emergency or stressful situations; and recognize reportable events

Knowledge, Skill and Physical Requirements:

  • BA/BS ( minimum) in social work, human services, or related field from an accredited program

  • A minimum of 3 years of work experience in social work and/or human services field

  • Must communicate effectively both verbally and in writing.

  • Computer skills with proficiency in Microsoft Office (Excel, Word, Outlook).

  • Must have a valid Florida driver’s license with reliable transportation and a good driving record.

  • Pass a Level 2 Background Screening with satisfactory findings

  • Resident of Seminole County
